CORE / Forms & Assessments

Flexible data collection without
breaking the common model.

Use XLSForm-based Forms and Assessments to capture programme-specific information while keeping it connected to CORE’s shared operational structure.

Forms are part of the workflow. Data collection is not isolated from operations: forms can stay connected to participants, implementations and programme context.

Field-ready collection

Structured forms without losing operational context.

CORE supports forms and assessments as part of day-to-day programme work. This helps teams collect rich information in the field while keeping the result linked to the participant, implementation or service it belongs to.

  • XLSForm-based logic familiar from Kobo and ODK.
  • Reusable forms for recurring operational needs.
  • Connected assessments, signatures and supporting documentation.
  • Cleaner inputs for reporting and follow-up.

01 / A common standard

Built on XLSForm.

CORE uses XLSForm, the same open form standard widely used with Kobo and ODK. Existing XLSForm knowledge and resources can therefore be reused rather than recreated around a proprietary form language.

01

Connect to CORE entities

Associate forms with Individuals, Households, Collectives, Facilities, Projects, Activities or Activity Implementations.

02

Use throughout operations

Capture assessments, implementation data, follow-up and programme-specific information.

03

Customise implementation

Use a dedicated form to extend an Activity Implementation with structured contextual data.

04

Work with Kobo / ODK

Use a familiar standard across field data collection and CORE.

02 / Flexibility without fragmentation

Adapt at the right level.

Not every information need should change the common data model. CORE offers several levels of contextualisation.

01

Standard CORE fields

The shared foundation of the model.

02

Configurable entity fields

Recurring contextual information can extend relevant entities while preserving the common structure.

03

Ad-hoc information

Add a field and value to a specific Participant or Activity Implementation when information is exceptional or record-specific.

04

Forms & Assessments

Use XLSForm for more complex structured data collection.

03 / Practical data collection

Capture more than text fields.

Forms can support operational collection needs including participant signatures. Through Kobo, a participant can sign directly on a phone or tablet using the signature field; an image can also be stored where appropriate.

A signed form connects participant confirmation to the record.
01

Participant signatures

Capture a signature through the Kobo signature field using finger or stylus.

02

Image upload

Store an image when that is the appropriate way to retain the information.

03

Reusable forms

Reuse or clone forms instead of rebuilding recurring structures.

Standardise the foundation. Keep the edges flexible.
